Who We Are
Streams of Hope works to assist individuals living with mental illness to reach a higher level of independence. Our Psychiatric Rehabilitation Program (PRP) assists participants with accessing and coordinating services and resources in their communities and works to ensure that each individual has access to the best possible treatment and care.
We offer rehabilitation services in community settings or the client’s home to assist clients with reaching their full potential.

Streams of Hope Eligibility Requirements
Streams of Hope accepts individuals who have been diagnosed with a mental health disorder and currently Working with a licensed mental health professional, such as a therapist or psychiatrist. If you are not actively working with a therapist, we will assist in finding you a licensed therapist.
Referral Process
- All clients we serve for PRP services are required to be referred by a licensed mental health professional.
- Upon receipt of referral and approval into the program, an assessment is performed in-home/in-office
- A treatment plan is developed based on the assessment
- A rehabilitation counselor is assigned to the client to begin services
